Provide person-centered care to improve residents' independence and quality of life through individualized treatment plans. Evaluate physical conditions, recommend adaptive equipment, and collaborate with an interdisciplinary team to monitor patient progress.
Requires an active and valid Texas Occupational Therapy license and strong clinical decision-making skills. Experience in Skilled Nursing Facilities (SNF) and familiarity with Net Health documentation are preferred.
