Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and prevent or minimize disability.
• Develop and implement plans to help patients reach maximum performance within their capabilities.
• Perform initial admissions/evaluations and, where appropriate, supervise Physical Therapy Assistants.
• Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans; monitor progress toward goals.
• Notify the assigned Clinical Team Manager of OASIS scores within established timelines.
• Coordinate care with patients, families, and other members of the health care team to align with care plans and patient goals.

Requirements

• One year of Home Health experience
• Ability to work effectively in a home health setting and manage patient-centered care
• Capability to perform initial admissions/evaluations and contribute to discharge planning
• Proficiency in documenting clinical findings, interventions, goals, and discharge plans with timely OASIS submission
• Flexible scheduling to accommodate patient needs, including a weekend requirement as described
