You'll provide skilled, compassionate care in a variety of settings, including private homes, nursing homes, and assisted living facilities.
This is a field-based position requiring travel throughout our service area.
Key Responsibilities Coordinate and deliver individualized care for hospice patients and their families Provide direct nursing care in alignment with the Interdisciplinary Plan of Care Perform ongoing patient assessments and adjust care plans as needed Collaborate with interdisciplinary team members to ensure optimal patient outcomes Coordinate and oversee care provided by vendors and community partners Participate in patient admissions and provide nursing services as needed outside of standard hours Take part in an on-call rotation, including emergency backup and occasional holiday coverage
Qualifications
Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (in good standing) Prior hospice experience preferred Ability and willingness to travel within the service area using a reliable, insured, and registered vehicle Experience with Point of Care EMR documentation Current CPR / Basic Life Support (BLS) certification Strong clinical, organizational, and communication skills Ability to work independently while collaborating effectively with a care team
