Establish and implement treatment plans based on physician orders for home health patients. Prepare clinical progress notes and educate patients and caregivers to meet individual therapy goals.
Must be a licensed Occupational Therapist in Oregon with a degree from an accredited program and CPR certification. A National Provider Identifier (NPI) is required upon hire, with home health experience preferred.
