The Clinical Pharmacist will lead clinical pharmacy services, focusing on chronic disease management and optimizing medication use through direct patient care. They will also oversee pharmacy operations, including prescription dispensing and 340B program compliance.
Candidates must be a licensed pharmacist in Virginia or eligible for licensure within 45 days. A PGY1 residency or equivalent experience is preferred, along with certifications in immunization and BLS.

We are seeking a self-starting, growth-oriented Clinical Pharmacist to join our team. The Clinical Pharmacist will support both dispensing and clinical care, with a primary focus on optimizing medication use and improving health outcomes through direct patient care, collaboration with the care team, and implementation of clinical pharmacy services. This role will evolve based on patient needs and the pharmacist’s professional interests, with opportunities to develop and expand services within chronic disease management, population health, and quality improvement initiatives. Essential Responsibilities In addition to traditional pharmacy functions, the Clinical Pharmacist is responsible for: Clinical Care & Patient Management Leading and expanding clinical pharmacy services, including chronic disease management (e.g., diabetes, hypertension, asthma), medication therapy management (MTM), and comprehensive medication reviews (CMRs). Collaborating with medical providers to identify and resolve medication-related problems, optimize therapeutic outcomes, and develop patient-specific care plans. Participating in interdisciplinary care teams and population health programs, including quality improvement and value-based care initiatives. Assisting in transitions of care by reconciling medications, educating patients, and reducing hospital readmissions. Conducting patient education and counseling on disease states, medication adherence, side effects, and lifestyle interventions. Administering immunizations and prescribing contraception under collaborative agreements and state protocols. Pharmacy Operations Overseeing prescription processing and dispensing, ensuring legal and regulatory compliance. Serving as a subject matter expert for drug information, formulary management, and clinical protocols. Supporting 340B program oversight, compliance, and optimization. Precepting Pharm.D. candidates and mentoring student interns or residents. Contributing to policy and protocol development, especially those related to clinical services and patient safety. Leadership & Professional Growth Identifying opportunities for innovation, expansion, and improvement in pharmacy services. Acting as a clinical resource for the pharmacy team and other departments. Participating in staff education, interdisciplinary rounds, and performance improvement initiatives. Demonstrating commitment to continuing education, board certification, and clinical training. QualificationsRequired Qualifications Licensed Pharmacist in the Commonwealth of Virginia (or eligible). Licensure required within 45 days of employment. Strong clinical background; PGY1 residency or equivalent experience preferred. Certification in immunization and hormonal contraception (within 6 months of hire). CPR (BLS) certification (within 6 months of hire). Demonstrated ability to work independently and proactively in a team-based care environment. Excellent communication, problem-solving, and organizational skills. Experience with underserved populations and value-based care is a plus. Physical Requirements Ability to lift or move up to 25 pounds and perform standard pharmacy-related physical tasks. Adequate hearing, vision, and speech to fulfill job duties in a clinical setting. Work Location This position may require travel to multiple HCHC sites.
