Job Description

The case manager will bear primary responsibility for the general case management of an assigned caseload of clients at SW Resources in Parkersburg, WV. Duties and responsibilities include, but are not limited to, the following: Responsible for completing assessment duties as assigned. Meet, orient, and instruct individuals to the assessment process. Prepare assessment reports, as well as daily documentation and case notes. Ensure individual service plans are completed during first 30 days, then annually. Monitor progress and behaviors of client employees on caseload. Provide linkage and referral for other services as appropriate or prescribed. Serve as an advocate for client employees. Prepare and present new referral during Admission and Placement meetings. Coordinate placement or job retention activities to include preparing and assisting individuals with job search, utilization of community resources, marketing, and consulting with employers. Participate in Quality Assurance of client employee records. Assist supervisors in phone calls regarding work. Assess Division of Rehabilitation Services (DRS) referrals in all phases of training. Assist clients with payroll onboarding through web-based HRIS system and answer any questions that may arise. Attend DRS meetings as needed. Serve as job coach when needed. Provide transportation for clients as needed. File client records as needed. Other duties as assigned. Qualifications Bachelor’s Degree with one year of related experience, or equivalent combination of education and experience required. Familiarity with the Division of Rehabilitation (DRS) organization, including DRS billing practices, preferred. Must have ability to obtain CPR and First Aid Certification within 30 days of hire. Must have ability to pass criminal background check. Valid driver's license required. Must be 21 years of age or older.

Additional Information

Beginning rate of pay is $18 per hour. Full-time position. Requires a Bachelor's degree with one year of related experience or an equivalent combination of education and experience. Candidates must have a valid driver's license, be at least 21 years old, and be able to pass a criminal background check.
