The role involves coordinating high-risk patient management and monitoring population health data to improve health outcomes. Responsibilities include engaging patients through motivational interviewing and preparing workflows to support team-based care.
Candidates should have a bachelor's degree or equivalent experience, preferably with 1-5 years in community health or patient coordination. Proficiency in English is required, while Spanish language skills are preferred.

As a population health coordinator, you’ll have an important role in preventive care and in addressing social drivers of health, and will work with patients toward improving health outcomes. Plus, you won’t be working weekends.
You will be primarily based at our Riverway location, 1569 Niagara Street, Buffalo.

$28.00 an hour based on a full time, 40 hour work week.
