Responsibilities
- Assist physical therapists in executing individualized treatment plans for patients with various musculoskeletal and neurological conditions.
- Conduct therapeutic exercises, manual therapy techniques, and modalities such as ultrasound or electrical stimulation under the supervision of a physical therapist.
- Monitor and document patient responses to treatment, reporting progress and any concerns to the supervising physical therapist.
- Educate patients and their families on exercises, proper use of assistive devices, and strategies to prevent injury or re-injury.
- Maintain a safe and clean treatment environment, ensuring all equipment is properly sanitized and in good working order.
Requirements
- Successful completion of accredited physical therapy program and state licensure exam.
- Valid Physical Therapy Assstant license
- Valid state driver’s license
- 1 year of home health experience preferred
- AHA CPR certification
- Valid Driver’s License
- Valid Auto Insurance
- HomeCare HomeBase experience preferred
- Physical Demands
- Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
- Must be able to lift, carry, pull, or push up to 100 lbs..

- Full-Time A “full-time employee” is one who is regularly scheduled to work 30 or more hours per week or completes the required productivity points per week as stated in the position agreement. A regular full- An employee is eligible for Company-sponsored employee benefits beginning the 1st of the month following the completed 30-day probationary period. Pay-Day The Company operates a bi-weekly payroll process, paid on Tuesdays (10 days in arrears), normally 26 times annually. If one of the paydays falls on a Tuesday, Company Holiday, the the employee will be paid on Monday. Working Hours To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation. Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours are 8:00 am to 5:00 pm (CST). Mileage The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage accurately and timely each pay period.
