Provide direct and indirect nursing care to participants through assessments, care planning, and clinical interventions. Coordinate care across clinic and home settings while collaborating with an interdisciplinary team to ensure high-quality, person-centered outcomes.
Must be a graduate of an accredited School of Nursing with a current Michigan RN license, CPR certification, and a valid driver's license. Preferred candidates have three years of nursing experience, including at least one year working with frail or elderly populations.

As a Community Care Registered Nurse (RN), you will provide both direct and indirect care to participants across the continuum, working independently under the guidance of the Director of Clinical Services or designee. In this role, you’ll use advanced clinical knowledge and judgment to manage complex situations, coordinate care, and take accountability for outcomes—all while being part of a supportive, collaborative team dedicated to delivering high-quality, person-centered care in a positive and balanced work environment.
Follow clinic workflows and participate in meetings and huddles as directed. Delegate nursing care appropriately and support team collaboration. Participate in Quality Improvement initiatives and promote safe, cost-effective care practices. Three (3) years of nursing experience in a healthcare setting. At least one (1) year of experience working with the frail or elderly population.
