The LVN Care Coordinator serves as the primary point of contact for patients, developing and monitoring comprehensive care plans within a multidisciplinary team. Responsibilities include providing nursing services, documenting encounters, and coordinating social service needs for a complex patient population.
Candidates must hold a current California Vocational Nurse license, a High School Diploma, and BLS certification. Proficiency in general computer skills is required, and bilingual fluency in English and Spanish is preferred.
