The Social Worker assesses patient and family needs to develop treatment plans and provide direct social services. They act as a patient advocate and liaison between the health center and community resources to address social determinants of health.
Provides orientation, training, and supervision to social/human services practicum students. The tasks outlined above are intended to provide a general overview of the role and are not an exhaustive list of all duties, responsibilities, or job functions. Qualifications Must have a Bachelor’s Degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Certified Social Worker (CSW) by Nebraska State Law is preferred upon hiring. If not certified at the time of hire, certification will be required within 90 days of employment. Excellent verbal and written communication skills; English/Spanish proficiency required. Task-oriented and detail-oriented with excellent customer service and organizational skills. Great computer proficiency with Microsoft Office and a willingness to learn other programs. Working knowledge of the Latino culture and commitment to cultural competence. Familiarity with social service resources in the community. Willingness to complete home visits as deemed necessary (Motor Vehicle Report (MVR) required) and a valid driver’s license. Prior experience in completing needs assessments and evaluations is preferred. Two years social work experience in a healthcare or social services environment is preferred. Functions well in a team environment. Motivated and self-directed. Experience working with Domestic Violence, Child Protective Services (CPS) and Adult Protective Services (APS) is a plus.
