The Case Manager serves as a bridge between clinical services and community resources for veterans and their families. They facilitate referrals for housing, healthcare, and legal needs while maintaining a network of trusted community partners.
Requirements summary
A bachelor's degree in Social Work, Psychology, or a related field is required, with a master's degree preferred. Candidates need at least one year of case management experience and must obtain a Behavioral Health Case Management II certification within six months.
The Case Manager is part of a multidisciplinary clinical team who serves as the bridge between clinical services and essential community resources.
The Case Manager facilitates direct referrals for housing, employment, healthcare, legal, educational, peer-support and all other essential community-based needs for clinic clients.
The Case Manager manages referral partnerships by continuous engagement post-referral, ensuring the provision of quality care.
The Case Manager assists the clinical team in understanding resources and needs within the clinic community.
The Case Manager is estimated to support ~250 clients a year (based on expected utilization levels).
Essential functions and responsibilities
Provides comprehensive case management to veterans, active-duty, guardsmen, and their families seeking mental health services
Develops network of trusted contacts within organizations in which to refer and coordinate care.
Creates and maintains an organized, evolving resource list covering all major areas of need, accessible to the team via Red Rock SharePoint
Establishes and maintains warm, supportive, professional relationships with clients
Provides appropriate referrals as necessary and assists clients in navigating systems of care
Consults with service providers and community partners on resource-related issues
Ensures confidentiality and controls access to sensitive information
Thoroughly and efficiently completes documentation within required timeline of 48 hours of intervention/communication
Collaborates with clinical team and participates in multidisciplinary team meetings
Follows care follow-up procedures to ensure clients receive services to which they are referred and identify/address any follow up needs Conducts intake assessments as needed
Provides crisis management support to clients and the team as needed
Provides services in-person and via telehealth platform, as required
Performs other duties as required

Proficient in use of technology to perform duties
Displays appropriate understanding of and adherence to policies and procedures and organizational hierarchy
Ability to set priorities and evaluate and create solutions to work related challenges
Complete Behavioral Health Case Management II certification within 6 months of hire
Must have a valid driver’s licenses and reliable transportation
Physical requirements
Must have the ability to stand or sit for long periods of time
Must have the ability to lift, push, or pull a minimum of 25 pounds
Ability to travel approximately 10% of the time
