Provide medical oversight, guidance, and direct care to adults with mental illness in a restorative respite program. Responsibilities include delivering health education, coordinating medical care, and performing clinical interventions based on individual action plans.
Candidates must hold a Bachelor's degree in Nursing and possess a valid Massachusetts driver's license. Experience working with adults suffering from severe mental illness is required.
Provide education and consultation to clients and staff on medical health, medications, and general medical topics Deliver one-on-one training tailored to specific individual needs Develop and lead groups and trainings for clients and staff on general health and wellness topics, including nutrition, exercise, and smoking cessation Assist clients in accessing community resources that support their health and wellness goals Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and available treatment options Perform direct medical care for a Person as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc. This includes providing interventions specific to Person’s IAP (Individual action plan) for medical/health and wellness goal areas. Review medical documentation for each assigned client and provide consultation and coordination of care as needed Coordinate overall medical care for clients and ensure necessary follow-up is completed Provide health and wellness interventions focused on education and skill-building Support the team and clients by introducing tools for ongoing symptom assessment, evaluating treatment effectiveness, and identifying appropriate responses or next steps
