Collaborates with a multi-disciplinary team to plan appropriate treatment and continuing care for patients. Assists patients and families in selecting and utilizing appropriate services during the discharge process.
Candidates must be fluent in medical terminology and hold a BLS Certification. Ability to interpret safety rules, operating instructions, and procedure manuals is required.

Plays an integral part in discharge planning of the patient and assists the patient/family in choosing/utilizing appropriate services. Responds to referrals.
Fluent in medical terminology. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. BLS Certification.
