The Discharge Planner provides essential services to patients and families across all hospital areas to facilitate smooth transitions of care. Key duties include case consultation, patient education, and resource management to meet discharge planning needs.
Candidates must possess a High School Diploma and at least 2 years of experience in acute care. A BCLS certification is required within 60 days of hire and must be maintained.
The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning. EDUCATION, EXPERIENCE, TRAINING High School Diploma and at least 2 years of acute care experience. . BCLS Card within 60 days of hire and maintain.
