Coordinate comprehensive care plans and serve as a primary advocate for patients in a home care setting. Conduct regular assessments, monitor patient progress, and provide health care instructions to patients and their families.
Requires an active North Carolina RN license and a minimum of 2 years of nursing experience. Candidates must have a reliable vehicle for travel and a degree from an accredited nursing program.
Active RN license in North Carolina or have a multi-state license. Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma. Minimum of 2 years of nursing experience (case management experience preferred). Demonstrated ability to read, write, and effectively communicate in English. Strong clinical skills and a deep understanding of case management principles. Excellent communication and patient advocacy skills. Prior home care experience strongly preferred, but not required. A passion for helping others and a compassionate, patient-centered approach to care. Solid computer skills: prior experience with electronic medical records (EMR) preferred. Must have own reliable vehicle Must be flexible Ability to travel to cases as assigned.
