The Ambulatory Care Nurse provides nursing care management and coordinates patient plans of care to maximize community-based living. They act as a communication link between physicians, patients, families, and interdisciplinary healthcare teams.
Requires a Registered Nurse license and either a Diploma or Associate's degree in Nursing, with a Bachelor's degree preferred. Candidates need 2 years of nursing experience, or no experience if they possess the preferred education.
