Overview Physical Therapist – Home Health role in Glendive, Montana.
Travel - Temporary Assignment starting 06/22/2026 for 13 weeks.
Guaranteed 38 hours per week with a weekly pay range of $2,199 to $2,249.
Day shift in a long-term care (LTC) nursing home setting.
This assignment is designed for a traveling PT providing rehabilitative services within a skilled nursing environment.

Responsibilities
Evaluate residents and develop individualized physical therapy plans of care for the LTC setting.
Implement interventions focusing on mobility, strength, balance, gait training, and fall prevention.
Administer therapeutic exercises, endurance programs, and assistive device training; monitor progression.
Document patient status, progress, and changes in the medical record; communicate with the care team.
Collaborate with nursing, therapy colleagues, and interdisciplinary team to coordinate care and discharge planning.
Educate residents and families on activity modification, ADLs, safety, and home program strategies.
Requirements
Active Physical Therapist license in Montana or eligibility for MT licensure
Doctor of Physical Therapy (DPT) degree or equivalent
Ability to work in a long-term care environment and participate in a travel assignment
Relevant ability to communicate effectively with patients, families, and care teams
Experience in geriatric or skilled nursing facility settings is preferred but not required
