Overview Physical Therapist – Home Health travel assignment in a long-term care setting in Bangs, Texas, United States. Start date 06/15/2026 for a 13-week assignment. Weekly pay range: $2,148–$2,275 per week. Guaranteed hours: 36 per week.

Responsibilities

Perform initial physical therapy evaluation and establish a plan of care for residents in the LTC setting.
Deliver evidence-based therapies to improve mobility, strength, balance, gait, and functional independence.
Reassess residents regularly and modify treatment plans based on progress and goals.
Document assessments, treatment interventions, and outcomes in patient records in a timely, compliant manner.
Collaborate with nursing staff, therapy colleagues, and care teams to coordinate care and support discharge planning as appropriate.
Educate residents and families on exercise programs, safety strategies, and Activity of Daily Living (ADL) optimization.

Requirements

Active Physical Therapist license in Texas (or eligible for licensure)
Degree in Physical Therapy (Doctor of Physical Therapy preferred)
Ability to work in a long-term care setting
Prior clinical experience in physical therapy; emphasis on mobility, functional training, and fall prevention
CPR certification or ability to obtain prior to start
Must be legally eligible to work in the United States
