Responsibilities

• Perform initial physical therapy evaluation and establish a plan of care for residents in the LTC setting.
• Deliver evidence-based therapies to improve mobility, strength, balance, gait, and functional independence.
• Reassess residents regularly and modify treatment plans based on progress and goals.
• Document assessments, treatment interventions, and outcomes in patient records in a timely, compliant manner.
• Collaborate with nursing staff, therapy colleagues, and care teams to coordinate care and support discharge planning as appropriate.
• Educate residents and families on exercise programs, safety strategies, and Activity of Daily Living (ADL) optimization.

Requirements

• Active Physical Therapist license in Texas (or eligible for licensure)
• Degree in Physical Therapy (Doctor of Physical Therapy preferred)
• Ability to work in a long-term care setting
• Prior clinical experience in physical therapy; emphasis on mobility, functional training, and fall prevention
• CPR certification or ability to obtain prior to start
• Must be legally eligible to work in the United States
