The Registered Nurse Case Manager coordinates medical treatment, schedules appointments, and oversees medication management for clients with acute medical needs. They provide medical education to staff and ensure all health records comply with government regulations and agency policies.
Candidates must hold an Associate or Bachelor of Science in Nursing and be a licensed Registered Nurse in the State of Texas. At least one year of experience in pediatric or psychiatric nursing is required.
The Registered Nurse Case Manager participates in treatment planning for clients with acute medical needs, schedules and coordinates medical appointments, reviews medical documentation, oversees medication ordering, refills, storage, and administration processes, communicates with the team about medical needs of each client, and conducts routine audits for all medication documentation. Ensures that all clients are being treated with the Psychotropic Medication Utilization Parameters for Children and Youth in Foster Care. The Registered Nurse Case Manager observes clients and assists in identifying and managing any side effects experienced due to medication.
The Registered Nurse Case Manager provides medical education for the agency to ensure medications and treatments for youth are conducted in accordance with approved techniques, licensing standards and physician instructions. The Registered Nurse Case Manager ensures that all medical records, logs, and medical appointment documentation are accurate and in the client file. The Registered Nurse Case Manager provides medically related functions necessary for the agency to fulfill the requirements of government regulations related to health and medication management.
Associate or Bachelor of Science in Nursing and currently licensed by State of Texas as a Registered Nurse.
At least one year of pediatric or psychiatric nursing experience.
Assess resident medical needs and coordinate appropriate treatment, follow-up care, and physician communication Schedule medical appointments for clients and work with team to ensure there is a plan to transport the client Attend psychiatric and specialty medical appointments with clients Oversee medication ordering, refills, storage, and administration processes to ensure that clients receive medication as ordered Administer medications as needed Review intake files of all potential clients with documented acute and/or long-term medical needs Work collaboratively with Program clinical team, Program operational team, and agency compliance team to meet the individual medical needs of clients and uphold all agency and DFPS policies, procedures, and guidelines Participate in treatment team meetings as a part of the treatment team for clients with acute, chronic, or specialized medical needs or medical diagnosis that require regular maintenance Provide training to staff on how to meet the medical needs of clients Interpret physician’s instructions to direct care staff and ensure that each treatment is administered as ordered Provide training and support to clients to support their knowledge of any medical diagnosis and understanding of their medication Conduct weekly audits of medication documentation, medical records, logs, and storage practices Utilizes agency documentation systems to maintain accurate and timely clinical records Consult with staff regarding nutrition, dietary concerns, and wellness-related issues Participate in 24-hour on call rotations to address medical concerns
Exposure to Exposure to bloodborne pathogens, bodily fluids, illness, and injury. Frequent standing, walking, bending, and movement throughout the workday. Ability to respond appropriately in crisis situations and utilize approved crisis intervention techniques when necessary.
