The primary responsibility of the Social Worker is to provide integrated behavioral health and care management services to patients and their families within the primary care clinic setting to effectively manage personal, interpersonal, and practical problems related to their health or behavioral goals.
The Social Worker will make appropriate use of community resources and evidence-based interventions to promote optimal independent functioning. The Social Worker is a core member of the collaborative care team which includes the patient's medical provider and psychiatric consultant, as well as the larger medical care team.
