Sina Health is seeking a compassionate, patient- centered Geriatric Physician to provide comprehensive medical care to older adults across multiple care settings. This role includes providing primary and chronic disease management services within skilled nursing facilities, personal care homes, assisted living communities, and the outpatient office setting.
This ideal candidate is committed to improving quality of life for older adults through proactive, evidence-based medical care, interdisciplinary collaboration, and strong relationships with patients, families and facility staff.
1. Provide comprehensive medical evaluations, diagnosis, treatment, and ongoing management of acute and chronic medical conditions in geriatric patients.
2. Conduct routine and follow- up visits within skilled nursing facilities, personal care homes, and assisted living communities.
3. See patients in the outpatient office setting for primary and geriatric care services.
4. Develop individual treatment plans focused on patient goals, quality of life, and functional status.
5. Collaborate with nursing staff, therapists, social workers, specialists, and family members to coordinate care.
6. Participate in care planning meetings and interdisciplinary team discussions as needed
7. Review laboratory results, diagnostic testing, medication regimes, and treatment plans.
8. Provide timely documentation and maintain accurate electronic medical records
9. Facilitate discussions regarding advanced care planning, goals of care, and end-of-life decision making when appropriate.
10. Respond to urgent clinical issues and provide guidance e to facility staff within hours of operation
11. Ensure compliance with all regulatory, clinical, and organizational standards.
1. Doctor of Medicine ( MD) or Doctor of Osteopathic Medicine ( DO) degree from an accredited institution.
2. Board Certified in Internal Medicine, Family Medicine, Geriatric Medicine, or a related speciality.
3. Active Pennsylvania medical license.
4. DEA registration and controlled substance prescribing authority.
5. Experience working with geriatric populations, long-term care facilities, assisted living communities, or post- acute care settings preferred.
