BAYADA Home Health Care is seeking a compassionate and dedicated Occupational Therapist (OT) to join our Hudson County Visits team. This position offers the flexibility of full-time or part-time employment while providing one-on-one care to adult and geriatric clients in the comfort of their homes throughout Hudson County.
As a Home Health Occupational Therapist, you will be part of a multidisciplinary team committed to helping clients maintain independence, safety, and quality of life while receiving care where they are most comfortable—at home.

Responsibilities
Perform home visits within designated geographic territories.
Complete comprehensive evaluations of patients' functional abilities, including ADLs, IADLs, mobility, cognition, communication, and safety.
Develop individualized treatment plans designed to maximize independence and functional outcomes.
Assess home environments and recommend adaptive equipment and modifications.
Collaborate with nursing and other disciplines to develop and implement patient-centered care plans.
Educate patients, caregivers, and family members on therapeutic interventions and strategies to improve safety and independence.
Accurately document patient assessments, interventions, and outcomes using electronic medical records.
Qualifications
Current New Jersey Occupational Therapist license.
Minimum of one year of recent experience as an Occupational Therapist.
Graduate of an accredited Occupational Therapy program.
Prior home health experience preferred but not required.
Strong clinical, organizational, communication, and problem-solving skills.

Base Pay, depending upon qualifications and subject to negotiation
