Provide site-based case management for adults experiencing chronic homelessness to help them achieve housing stability. Responsibilities include conducting assessments, developing housing plans, and providing eviction prevention and re-housing support.
Requires a High School Diploma or GED and at least one year of experience working with individuals experiencing homelessness. Must possess a valid driver's license and an understanding of Housing First and Harm Reduction principles.
This is a 32/hr. a week full-time position. Qualifications What You Offer High School Diploma or GED required; Associate degree or higher preferred. One or more years of experience working with individuals experiencing homelessness. Lived experience is valued and considered toward experience requirements. Understanding of Housing First, Harm Reduction, and Trauma-Informed Care. Strong communication, documentation, and advocacy skills. Comfort working in unpredictable environments with a flexible, strengths-based mindset. Valid driver's license, insurance, clean driving record, and reliable transportation.
