The Medical Case Manager coordinates care and resources for patients within a primary care clinic, serving as a central point of contact between providers and community organizations. They develop individualized care plans, conduct assessments, and provide emotional and social support to optimize health outcomes.
Requires three years of experience in patient-facing healthcare or social service roles, specifically with vulnerable populations. An undergraduate degree in social work, psychology, or a related field is required, though experience may be substituted.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Work Experience: Experience working with vulnerable or underserved populations. Three years' experience in patient/client-facing roles in healthcare or social series. Experience in a primary care or safety-net clinic setting preferred. Education, Certification and Licensure: Undergraduate degree in social work, psychology or related field; experience may be substituted on a year-by-year basis. Additional Skills & Knowledge: Bilingual preferred.
