• As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting.

Schedule

• Monday- Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone

• On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.

Requirements

Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA). Unrestricted DEA licensure and prescribe authority (per state practice regulation). At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. Willingness to obtain additional licensures if needed based upon business needs. You have excellent communication and interpersonal skills. You’re dedicated to staying current with the latest developments in primary care and obesity medicine. You believe in inclusive, compassionate care. You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals. You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame. You’re willing and excited to try new technologies that are both patient and provider oriented. You are excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff.
