Key Responsibilities Collaborate with the clinical team to support the management and delivery of patient care plans Complete charting and documentation within required deadlines Self‑manage daily schedule, travel, and patient visits effectively Maintain a high standard of professionalism, communication, and patient engagement
Travel Expectations Ability to spend approximately 20% of the workday driving to and from patient homes
Schedule Monday – Friday, 8: 00 AM – 5:00 PM No weekends or holidays
