Overview Join our client’s compassionate care team as a Home Health Physical Therapy Assistant, supporting patients directly in their homes throughout Lebanon, PA, and nearby communities. This role offers a supportive environment with mentorship, flexible scheduling, and opportunities for professional growth. You will play a key role in delivering and managing patient care plans while collaborating closely with a dedicated clinical team.

Key Responsibilities Collaborate with the clinical team to support the management and delivery of patient care plans Provide care to patients in Lebanon, PA and surrounding areas Complete charting and documentation within required deadlines Self‑manage daily schedule, travel, and patient visits effectively Maintain professionalism, communication, and patient‑centered care at all times

Travel Expectations Ability to spend approximately 20% of the workday driving to and from patient homes

Schedule Monday – Friday, 8: 00 AM – 5:00 PM No on‑call, weekends, or holidays

Requirements: Required Qualifications

Associate degree or higher 1+ year of experience in ANY of the following

• Home Health Care Home Health 1+ year of experience in
• ANY of the following: Electronic Medical Record (EMR) systems Practice Partner EMR EHR Implementation Valid Pennsylvania Driver’s License Active Physical Therapy Assistant (PTA) License in Pennsylvania Has not applied to or worked for Residential Healthcare Group within the last 6 months Comfortable with required travel for the position
