Physical Therapist (Home Health)

• As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning.
• You will also be involved in patient and caregiver education, supervision, and reporting on patient progress.
• Key Responsibilities · Perform Physical Therapy assessments and develop treatment programs · Record and report on the patient's reaction to treatment and condition changes · Instruct patients and caregivers in therapy programs and device use · Supervise physical therapy assistant and, when appropriate, the certified nursing assistant · Prepare reports and participate in staff and Agency development activities Schedule and Shift Details · Schedule: Monday – Friday, day shift (no weekends, on-call, or holidays).
• Typically sees 5-6 patients per day.
• Travel · Local travel to residential homes

Requirements

Required Master's degree or higher Healthcare Setting (1+ years) ALL of the following valid licenses/certifications: Physical Therapist in Kansas (KS) Physical Therapist in Missouri (MO) Valid Regular Driver License (DL) Preferred Please provide your PT license number.
