Summary
Provide community-based respite support to individuals with extraordinary support needs. This role focuses on enhancing quality of life by promoting independence, community participation, and meaningful daily experiences while ensuring health, safety, and person-centered care.
Key Responsibilities
Deliver respite services in alignment with each individual’s Individualized Support Plan (ISP)
Support individuals with daily living skills, personal care, and community integration
Foster independence, self-advocacy, and informed decision-making
Provide transportation to community activities, appointments, and services as needed
Monitor individual progress and communicate updates to the team
Complete accurate and timely documentation, including service notes and medication records
Build and maintain positive relationships with individuals, families, and community partners
Support individuals in leisure, recreational, and social activities
Provide crisis intervention and emergency support when necessary
Qualifications
& Skills:
Strong interpersonal and communication skills
Ability to demonstrate empathy, patience, and professionalism
Ability to work independently and as part of a team
Understanding of person-centered practices and advocacy
Ability to support individuals with diverse needs and abilities
Physical & Work Requirements
Ability to work a flexible schedule, including evenings, weekends, and holidays

Frequent standing, walking, bending, and lifting

May be required to provide support in other Community Living programs (e.g., Supported Living, In-Home Services) as needed
Valid driver’s license, reliable transportation, and proof of insurance

CPR, First Aid, and Bloodborne Pathogens certification
Certified Restraint Training
Core A & B certification
Completion of required agency and regulatory trainings (e.g., Human Rights, Safety, Medication Administration)
