Deliver compassionate, high-quality care to patients across their lifespan while partnering with providers for coordinated treatment. Responsibilities include documenting care in the EHR system and participating in team-based quality improvement initiatives.
Candidates must hold an active CNA certification in North Carolina, a HS Diploma/GED, and current CPR/BLS certification. A valid driver's license is required, and clinical experience in primary or community health is preferred.
