The Licensed Practical Nurse (LPN) is a key member of the multidisciplinary treatment team, providing compassionate nursing care to individuals recovering from substance use and co-occurring mental health disorders. Working under the supervision of an RN, Nurse Manager, or Director of Nursing, the LPN assists with medical coordination, medication administration, patient monitoring, and clinical documentation while helping maintain a safe, structured, and therapeutic treatment environment. This role is essential to promoting patient safety, supporting recovery, and ensuring high-quality clinical care throughout the treatment process.
Clinical Care & Support Provide direct nursing care and support under the direction of an RN or nursing leadership. Conduct basic nursing assessments and promptly communicate significant findings. Administer medications and treatments in accordance with physician orders and facility protocols. Monitor vital signs, withdrawal symptoms, and patient stability. Observe patients for changes in condition and escalate concerns appropriately. Assist with patient education and discharge planning activities. Care Coordination & Collaboration Collaborate with nursing, medical, clinical, and recovery support staff to coordinate patient care. Participate in interdisciplinary treatment team meetings. Communicate observations regarding patient progress and clinical concerns. Support continuity of care and treatment planning efforts. Medication & Inventory Management Administer medications accurately and safely. Maintain Medication Administration Records (MARs). Monitor medication inventory and assist with ordering and reconciliation processes. Ensure medications are stored and secured according to regulatory requirements. Documentation & Compliance Complete accurate and timely clinical documentation. Maintain compliance with state, federal, accreditation, and payer requirements. Follow infection prevention and control practices. Support facility readiness for surveys, audits, and regulatory reviews. Therapeutic Milieu Support Help maintain a safe, structured, and recovery-focused environment. Support crisis intervention and de-escalation efforts. Assist patients in navigating the daily treatment schedule and program expectations. Perform additional duties and special projects as assigned.
Active, unrestricted Licensed Practical Nurse (LPN) license in the State of West Virginia. Graduate of an accredited Practical Nursing program. Current CPR certification or ability to obtain within 30 days of hire. Ability to work effectively within a multidisciplinary treatment team. Strong communication, organizational, and documentation skills.
One (1) to two (2) years of nursing experience in behavioral health, substance use disorder treatment, psychiatric care, detoxification, or residential healthcare. Experience working in a trauma-informed and recovery-oriented environment. Familiarity with withdrawal management and medication-assisted treatment (MAT). Experience with electronic medical records and clinical documentation systems.
