M.D., D.O. · Completion of residency and Allergy & Immunology fellowship · REQUIRED LICENSE OR CERTIFICATION · Board Certified or Board Eligible in Allergy/Immunology · Unrestricted licensure in the state of Florida · Active and unrestricted DEA License
