The nurse coordinates outpatient services for pediatric cardiology patients, managing clinic flow and acting as a liaison between families and healthcare providers. Responsibilities include performing clinical procedures, handling insurance authorizations, and ensuring the timely coordination of patient care.
Requires a BSN, current California RN licensure, and at least five years of pediatric cardiac inpatient nursing experience. Mandatory certifications include ACLS, BLS, and PALS.
Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Pediatric Heart Center Nurse works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices.
The Pediatric Heart Center Nurse Care Coordinator is responsible for coordinating outpatient services and needs for a panel of pediatric cardiology patients and providers in the ambulatory setting. His/her responsibilities include both in clinic procedures, managing the Outpatient Heart Center clinic flow as well as assisting with insurance authorization and prior medication authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care. This position is 100% in-person onsite. This position support patient care primarily in San Francisco and as needed in Oakland and satellite clinics.
