The pharmacist will improve medication-related outcomes for patients with osteoporosis through collaboration with multi-disciplinary medical teams in ambulatory settings. Responsibilities include comprehensive medication management and supporting endocrinology and orthopedics with infusion patients.
Candidates must have a pharmacy degree from an accredited college and a current Minnesota pharmacist license, with a Wisconsin license obtained within six months. Required experience includes two years of recent clinical pharmacy experience in ambulatory care or completion of a specialized post-doctoral residency.

The purpose of this position is to improve population based and patient specific medication related outcomes through collaboration with a multi-disciplinary medical team in the ambulatory setting. The pharmacist assumes a primary responsibility and accountability for the provision of safe and effective drug therapy and other medication management services intended to achieve outcomes related to the mitigation or prevention of disease, reduction of patients’ symptoms, prevention of adverse drug events and improvement of patients’ quality of life.
This pharmacist will be supporting endocrinology and orthopedics with patients who have osteoporosis and need infusions. The pharmacist will practice comprehensive medication management in addition to specializing in bone health.
• (BCACP) or Board Certified Pharmacotherapy Specialist (BCPS) preferred.
