Care Management Registered Nurse

A Care Management Registered Nurse works and assists patients and families in understanding patient conditions and treatments. They can help patients and family members develop coping strategies, assess patient needs, assist in the development of treatment plans and provide emotional and mental assessments on the patient.

Qualifications

Duties: And responsibilities:

• 1.
• Maintains weekly scheduled appointments in our electronic records systems called Epic.
• 2.
• Provide therapeutic services to patients and their families.
• 3.
• Work in collaboration with multidisciplinary team members including external partners.
• 4.
• Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner 5.
• Demonstrates cultural competency and commitment to patients.
• 6.
• Demonstrates commitment to the philosophy of Leon Medical Centers.
• 7.
• A Care Management Register Nurse is responsible for handling several tasks in order to provide the best service to patients 8.
• Provide Support to Patients and family 9.
• Help patients understand, cope with and treat their condition, illness or disease.
• 10.
• Participate in treatment plan development, arrange for in-home care personnel and equipment and assist in the scheduling of follow-up visits.
• Educate and Counsel Families 11.
• Notifies physician and Care Management supervisor of unusual reactions and/or changes in patient’s condition.
• 12.
• Documents all appropriate observation and treatments.

Qualifications

• Graduate of an approved school of nursing.
• Currently licensed in the State of Florida as a Register Nurse.
