About the role

The LPN Case Manager will join a patient-centered clinical team to manage case services and social services. The role focuses on providing high-quality medical coordination within an acute healthcare setting.

Hocking Valley Community Hospital (HVCH) is currently seeking a Licensed Practical Nurse (LPN) Case Manager to join our friendly, patient-centered clinical team for the following shifts: Case Management/Social Services, Full Time, Days, M-F Pay starts at $20.81/hr to $29.03/hr based on experience

Our Desired Candidate Will Have Graduate of an accredited program in nursing. Active State of Ohio Licensed Practical Nurse Nursing License. Requirements

Candidates must be a graduate of an accredited nursing program and hold an active State of Ohio LPN license. At least 5 years of nursing experience in an acute healthcare setting is required.
