Evaluate and treat clients with physical disabilities in a home care setting using exercise, manual therapy, and stretching. Collaborate with a multi-disciplinary team to establish treatment plans that improve client mobility and independence.
Must hold a current Minnesota Physical Therapist license and have graduated from an accredited physical therapy program. Experience in home care and proficiency with electronic medical records are preferred.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary team that provides rehabilitative care to clients, allowing them to live independently and age in place safely. At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. Our Care Management Model ensures that all team members work together to service our clients. Teams meet on a regular basis to answer questions, provide support, and stay connected.

Evaluate and treat clients suffering from physical disability due to injury, disease, or surgery in a home care environment Provide treatment for clients using a variety of methods including exercise, hands-on therapy, stretching maneuvers, and equipment to ease clients' pain and help them increase their mobility Test, measure, and assess clients' current condition and abilities Assist patients with all phases of Physical Therapy treatment and techniques to include exercise instruction, manual therapy techniques, activities of daily living, functional/total body progressions, modalities, and a fall prevention program Establish treatment plans specific to the client's specific needs that allows for excellent client outcomes Make recommendations for devices or tools that might improve the client's quality of life Work cooperatively and consistently with physicians, case managers, nurses, occupational therapists, and caregivers Provide training to team members as needed Request interventions to help avoid unnecessary trips to the emergency room or readmissions Monitor, document, and report clients' progress
