Facilitate positive health behaviors in adolescents with congenital heart disease using theory-based lifestyle interventions. Conduct assessments, develop intervention plans, and collaborate with multidisciplinary professionals to provide comprehensive care.
Requires a bachelor's degree in a health-related field such as exercise physiology, nursing, or psychology, and must be pursuing an advanced degree. An interest in working with adolescents is required.
Summary: Works with individuals, groups, or communities to facilitate positive and healthy behaviors. Interacts with adolescents who have congenital heart disease using a theory-based lifestyle intervention to increase physical activity.
Essential Functions: Conducts assessments to identify behaviors and develop intervention plans. Monitors and evaluates the effectiveness of interventions and modifies plans as needed. Collaborates with other professionals, such as psychologists and social workers, to provide comprehensive care. Provides training and support to caregivers and other professionals to ensure consistency in intervention strategies. Maintains accurate and up-to-date records of client progress and communicates with relevant parties as needed. Maintains accurate and timely documentation of all interventions and progress. Education Requirement: Bachelor’s degree in a relevant area, including exercise physiology, nursing, physical therapy, psychology, community wellness, social work, or another health-related field, required.
