How you’ll make an impact in this role Perform initial needs assessments to ensure individuals are guided to the most appropriate health services. Provide informal counseling, health education, and social support, while assisting patients with medical appointment scheduling and program applications. Manage access to health-related services by accompanying patients to appointments and ensuring they effectively utilize available community resources. Maintain precise patient records by documenting all contacts, services rendered, and clinical outcomes to support future reference and service tracking.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
