The provider will deliver a mix of virtual primary care and in-person extended care services for pediatric patients. Responsibilities include performing physical exams, managing chronic diseases, and documenting patient encounters in medical records.
Candidates must have a Master's or Doctorate in Nursing, a valid RN license, and at least 3 years of experience in pediatric primary care. Required certifications include BLS and a DEA license, with board certification in pediatrics preferred.
This position is a hybrid position and includes work on both the VPC and EC platforms. Virtual Primary Care (VPC): This practice is staffed by Advanced Practice Clinicians, supported by the physician team. These APPS may also provide in-office visits as coverage for providers outside of the office. Patients can schedule directly with their provider, preserving the trusted provider-patient relationship found in traditional practices on this virtual platform. Nutrition counseling, Parent education, Virtual house calls, Chronic disease management, and Behavioral health consultation are services offered by the virtual primary care practice. In person care is provided through Extended Care Clinic offering full primary care services in the evenings and during weekends. Extended Care (EC): In the evenings and on weekends, APPs will transition from VPC to providing shared IN-PERSON after-hours care Friday and every other Saturday and Sunday. In a weekend position the APP will work on VPC during the day and then transition to the Extended Care site from 5p-9p for in person visits on Friday. Saturday and Sunday work will be alternating on the VPC platform remotely and the Extended Care in-person location. The provider will therefore be able to have a mix of both virtual/remote and direct patient care. The Extended Care location will be open from 5-9 pm M-F and 8a-8p on Sat and Sun. Pay Range $47.50 - $71.25 Essential Functions Performs initial and periodic medical history, physical examinations, and daily assessments. Records findings of examinations, assessments and plan of care. Performs minor office-based medical and/or surgical procedures. Writes appropriate prescriptions and administers medications. Orders and interprets daily laboratory and radiology examinations. Determines treatment plan for patient. Documents all patient encounters, records observations, interventions, and actions in a medically appropriate manner on the patient's medical record. Acts as a patient advocate for the population served. Attends, participates and contributes to group/practice meetings and development of policies and procedures when requested. Physical Requirements Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English fluently and in understandable terms. Must possess intact sense of sight, hearing, smell, touch, and finger dexterity. Must have the ability to react and perform in stressful situations Education, Experience and Certifications. Must have or be eligible for a DEA license. Approved to practice as a Nurse Practitioner in the state one works. Eligible for prescriptive authority application through the Board of Nursing or Medical Board in SC required. BLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice. Demonstrates appropriate critical thinking, interpersonal skills, and the ability to prioritize and concentrate. Nurse Practitioner Master's Degree or Doctorate of Nursing Practice required. License to practice as a Registered Nurse required. Certification in the area of practice from a nationally recognized certifying body required. ACNP, AGACNP, AGNP, PNP, or PNP-AC board certification preferred based population served.
