Provide high-quality outpatient physical therapy to post-operative patients within their own homes. Develop detailed treatment plans and maintain continuous communication with referring physicians and patient families.
Requires a Bachelor's degree from an accredited Physical Therapy program and current PT licenses for Indiana and Illinois. Must possess CPR certification, a valid driver's license, and reliable transportation for home visits.
The Physical Therapist – Provides outpatient quality physical therapy within the patient’s own home and acts as a liaison for physician practice to meet the therapy needs of patients. The position is best suited for an outgoing and self-motivated therapist. The position offers a high level of flexibility as well as autonomy to take ownership of a practice and offers a new standard of post-operative care. The position will afford the therapist to spend one on one time with patients and also the latitude to help grow a new service line.
