The Case Manager works directly with children and families in the community to monitor needs and ensure continuity of care. Responsibilities include completing assessments, developing care plans, and collaborating with treatment teams.
A Bachelor's or Master's degree in human services, social work, or counseling is required. Candidates must possess a valid driver's license and a clean driving and criminal record.
