About the role

Manage a caseload of individuals with chronic homelessness by monitoring progress toward treatment goals and conducting home visits. Coordinate medical, mental health, and behavioral health services while assisting clients with budgeting and medication monitoring.

BronxWorks provides permanent supportive housing with an array of social services to more than 400 formerly chronically unhoused individuals and families throughout the Bronx. We strive to break the cycle of homelessness with personalized, wraparound support, empowering individuals and families to rebuild their lives with stability, dignity, and hope.

Qualifications

  • (education, professional certifications or licenses, experience, proficiencies)
  • Proficiencyin modern business communications including personal computers, electronic mail, voicemail,facsimileand copier equipment
  • Bachelor’s degree in social work or another related field ofstudy
  • One year of case management or relevant work experience preferred
  • Engagement and detailed oriented skills are essential
  • Proficiencyin Microsoft Office suite and other standard business technology isrequired
  • Strong oral and written communication, timemanagementand organizational skills
  • Essential Duties and Responsibilities
  • Maintaining a caseload of 14-18 individuals with chronic homelessness history
  • Monitor and document client progress towards Treatment plan goals
  • Conduct regular case conferences with clients and service providers as needed
  • Conduct monthly home visits for caseload and field work with clients when needed
  • Assistwith managing and directing allwalk-inclients
  • Maintain familiarity with agency and city resources available to clients
  • Document progress notes for all encounters and services provided to clients
  • Participate in regularmeetings, trainings and weekly supervisionwith supervisor and other support staff
  • Opportunities for clinical work through intensive case management, crisiscounselingand substance abuse counseling
  • Opportunities to provide group counseling
  • Opportunities to earn LCSW and LMCH qualifying licensing hours
  • Performadditionalduties as assigned
  • Program Specific Responsibilities
  • Completing ongoing Treatment Plans and Assessments for caseload
  • Coordinate and connect clients to variety of medical, mental health and behavioral health physicians and services
  • Assessing ongoing eligibility for various entitlements and resources for caseload
  • Meeting monthly reporting requirements includingtimelydocumentation and engagement with full caseload
  • Assistingclients with Representative Payee services including budgeting and financial education
  • Assistingclients with Medication Monitoring on a daily, weekly, or as needed basis
  • Skills/Abilities
  • Strong writing and oral communication skills
  • Strong presentation skills and ability torepresentthe agency at functions and meetings
  • Ability to collect and analyze client demographics and outcome
  • Excellent organizational and teambuilding skills
  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit forlong periodsof time
  • Ability to climb five flights of stairs, ifrequiredto conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, ifrequiredto conduct home visits and/or field work

Requirements

Requires a Bachelor's degree in social work or a related field, with one year of relevant case management experience preferred. Must be proficient in Microsoft Office and capable of traveling throughout New York City for fieldwork.
