The LPN will perform all health monitoring and nursing services for participants in an Adult Day Healthcare program. Responsibilities include coordinating admissions, maintaining medical records, and providing health education to participants and their families.
Candidates must be a graduate of an accredited nursing program with a current Florida LPN license. At least one year of experience working with individuals with disabilities, the aging population, or Alzheimer's patients is required.

Assist in scheduling in-house medical specialty visits, medical/health appointments, and transportation as needed Track, review, and coordinate quarterly care plan meetings with participants and family/caregiver team Take, document, and maintain records for each participant, reporting trends outside of parameters, or needing attention from the Center Director Provide clinical support, insight, knowledge, and applicable in-service training to program staff. Provide education to participants and family/caregiver team on health and wellness, medical diagnosis', medication, safe living, emergency situations, and resources available.
Graduate of an accredited nursing program Current Florida Licensed Practical Nurse License At least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patients Ability to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc) CPR and first aid certification a plus, but can be obtained/renewed once hired Easterseals Florida has been rated a Great Place to Work (2020-2025) by our employees!
