  • The Nurse Practitioner is responsible for the provision of preventive medical, palliative, and educational services to promote patient health and wellness and uses standard software and the comprehensive clinical-financial computer system ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
  • Essential Activities and Tasks Care Management - 100% Conducts comprehensive medical and social histories and performs physical examination of healthy, acutely ill, chronically ill patients or terminal patients.
  • Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
  • Orders and interprets lab tests and x-rays.
  • Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care and medication use.
  • Refers patients to other health professionals as needed.
  • Communicates with staff members, patients and families to promote optimal patient outcomes.
  • Completes documentation accurately and timely while maintaining state, federal and agency regulation.
  • Participates in quality improvement projects.
  • Commitment to excellent customer service for internal and external customers.
  • All other duties as assigned.
  • Qualifications Education Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP) required.
  • Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe.
