Provide comprehensive support and interventions for adults with Serious Mental Illness through home and community-based services. Coordinate care with medical providers and develop individualized service plans to monitor and improve member mental status.

Provides direct services in the home, community or office.
Interventions may be 1:1 or group based.
Performs crisis stabilization functions.
Assists member/ parent/guardian in resourcing with the community.
Conducts assessments and completes necessary paperwork.
Serves as central support for members.
Performs case management functions as needed.
These functions may include behavior coaching, member medication monitoring, clinical care coordination between resources within and outside the agency.
This position is a Monday- Friday schedule, flexibility to work during clinic operating hours, reporting to our Payson Outpatient Clinic.

Requires a minimum age of 21 and a combination of education and experience totaling 5 years, or a bachelor's degree with one year of behavioral health experience. Must possess a valid AZ driver's license and be eligible for fingerprint clearance.
