The Hospice RN will provide direct patient care and comprehensive case management for individuals receiving hospice services. This includes developing and implementing care plans, monitoring patient conditions, and supporting families.
Candidates must be graduates of an accredited nursing program with a current RN license in Connecticut and at least 2 years of nursing experience, preferably in hospice or home health. Strong clinical judgment, communication skills, and the ability to work independently are essential.
Graduate of an accredited nursing program; BSN preferred Current RN license in the State of Connecticut or Compact license. 2+ years of nursing experience; hospice or home health experience strongly preferred Ability to work independently and respond effectively in crisis situations Strong clinical judgment and interpersonal communication skills Comfortable with electronic health records and mobile documentation Valid driver's license, reliable transportation, and willingness to travel within service area/territory Current CPR certification Must complete RVNAhealth's hospice training program upon hire
