Key responsibilities

Administered medications and treatments as prescribed. Monitored resident conditions, vital signs, and reported changes to the healthcare team. Provided direct patient care, including wound care, catheter care, and assistance with daily living activities. Maintained accurate documentation and collaborated with interdisciplinary teams to ensure quality resident care. The LPN would report directly to the Charge RN,and would be responsible for reporting any change in condition to the RN for follow up.

Requirements

Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. CT LPN License CPR/BLS
